To create a new folder, from the PhysioGraphic toolbar, double-click on new, and the following dialog will appear right over the work area. From the questionnaire submitted to the patient, the office staff or practitioner fills out the form shown below:

This dialog box shown above is divided in several sections or property pages, listed as follows:

Patient profile section gathers all information pertaining to the identity of the patient. (First name, last name...)

Home section gathers all information pertaining to the address of the patient.

In the employer section, all information relevant to the employer may be found

In Referring physcian section is defined the physician that refers the patient to your healthcase center.

Paralegal section gathers all information pertaining to the paralegal business if any.

In Extended health benefit section, information  regarding the employer's insurance group if any.

In Automobile insurance section is specified  the auto-insurer if the patient has been injured by a car accident.

In Work-related injury insurance section is specified  the insurer  if the patient has been injured at work.

In Notes section, you may enter any valuable information that could not be stored in all other sections.

In Statistic section is defined the fees charged to the patient during an evaluation, assessment, treatment...

Finally in Reason for treatment section, you may enter the diagnostic and the cause of accident, as well as the injury date, and initial evaluation date.

Of course, the office staff doesn't have to fill all the sections, only the patient ID is mandatory and it must be unique.

The only point to highlight during the registration of new patient deals wth the databases for automobile insurances, paralegal businesses, referring physician, extended health benefit, work-related injury insurance. Let us click on extended health benefit tab, the dialog box will look as follows

First you need to register the name of the business offering the Extended Health Benefit, if not already registered. Once the business registered, you must select it in control labeled with "None selected". As soon as you have made the selection, PhysioGraphic fills all the form with the default data (name, address...) of the business. Afterward, the user is free to modify any information, except the  business name offering the Extended Health Benefit. As you will see later on, PhysioGrapic allows you to create charts over your referrals.

At last, some pages are provided with search functions  to speed-up your selection of your insurers and physicians. To perform a search, click on and the dialog box below pops up on your screen:

Please enter your search string then click on the Find button. PhysioGraphic will select the first occurence of the string. For the next occurence, please click on .
