First click on Folder in the PhysioGraphic toolbar if the active tab is labeled as either Clinic, Evaluation, or Report. Then double-click on the subtree Document to expand it.Then double-click on questionnaire, or on ailing man in blue jacket. PhysioGraphic prints a questionnaire to be handed to the patient.

Once the patient fills out the questionnaire, the office staff or the physical therapist is ready to register the new patient to PhysioGraphic.
